1. Select a Plan:

Delta Dental Premier (Formerly known as DeltaPremier)	Delta Dental PPO Plus Premier (Formerly known as Delta Preferred Option Plus)	Delta Dental PPO (Formerly known as DeltaPreferred Option)
DeltaCare	The Value Plan	Delta Dental PPO Value Plan* (Formerly known as Preferred One)
*Type I services are covered 100% when receiving care from a PPO dentist. Type II and III services are discounted when a member receives services from a PPO dentist.

2. Enter Location:

Zip Code: (at least 3 digits required; for example: '021', '0211', or '02116')	(for use with the Zip Code feature only, a 5 digit Zip Code is required.)

City:	State: MA

3. Select Specialty:

If you are a Delta Dental PPO member and are looking for an Orthodontist, please select Delta Dental PPO as your plan and then Orthodontics as the specialty to locate a participating Orthodontist.

4. Enter Dentist's Last Name / Practice Name:
(optional; to search for a particular dentist / practice)

(for example: 'SMITH' or 'SUPER DENTAL, INC.')
If you are looking for a dentist or practice and are not sure how to spell the name please use a "%" in front of and behind your search word. For Example, "%smi%" will display a listing of dentist names and practices that contain "smi" such as Smith, Smithfield and Desmitto.

5. Enter Street Address:
(optional; to search for a particular street address)

(for example: "Medford Street")

Our on-line directory will help you locate a network dentist near you. You can also search for a specific dentist by name. By receiving care from a network dentist, you'll lower your out-of-pocket costs and eliminate the need to file claim forms because our network dentists will do this for you.
