Fill out the
Dentist’s Enrollment Form and fax it, e-mail it or mail it to us.

We will send out an application to you via fax, e-mail or mail according to your request.

An enrollment coordinator will be assigned to you to assist with the process and answer any questions.

We will work closely with you to ensure credentialing and enrollment is a straightforward and easy process.

Upon completion of all application requirements a welcome letter will be mailed with your effective date.
