Tax Identification Number

Delta Dental of Massachusetts is required by the Internal Revenue Service (IRS) to report all payments made to you annually. The amount is reported on Form 1099 under a tax identification number (TIN) or social security number that should match the IRS's records. The IRS now requires that only one individual or business name may be associated with a tax identification number (TIN) or social security number.
